More specifically, we regularly seek to understand the core facts underlying any potential claims you have may have from any job you’ve worked at within the past three (3) years, including:

  • your dates of employment, position held, essential job duties, rate of pay, regular days and hours worked, average gross weekly wages, who you reported you, and the names and positions of other employees who will have knowledge of your duties & hours worked;
  • the documents that relate to your claims including pay stubs, time sheets, schedules, W-2 forms, employee handbook, etc.; and
  • based upon the initial information you’ve provided to us, how any claims may be calculated, subject to verification through witnesses and other evidence.
